Bridging the gap: Improving hepatitis care for patients released from correctional facilities

BCCDC and BCMHSUS  aim to improve access to HCV care among people released from correctional facilities through the Test, Link, Call project (TLC project). The project will ensure that:

1. Patients are connected to healthcare providers in their community
2. Patients receive the needed support from people who have similar experience during their transition
