Leaving custody can be a very stressful experience, and there are often limited resources to support people to successfully make this transition. It can also be difficult for people who are supporting someone else who is leaving custody to know how to get them help.
People who have experienced incarceration share that planning before release can help make it less overwhelming when someone leaving custody goes back to the community.
To help make this process easier, Peer Health Mentors with Unlocking the Gates (UTG) Services Society put together checklists and a list of resources. Peer Health Mentors with UTG are all people who themselves have previously been incarcerated, and they have made the transition from custody back to the community themselves, and have supported 100’s of other people to make this transition successfully.
