As the TLC project has continued to organically expand to meet the needs of partners, the ability to manually collect quantitative data from sites on outcomes achieved among their clients has become unsustainable. To address this, the development of a cloud-based solution with a web application interface has been initiated, called the ‘Bridge2Care’ (B2C app). This application will initially support client enrollment and data capture, in addition to obtaining client consent for administrative data linkage. The app is being created by Thrive health. The first iteration of it is expected to be deployed in the field by TLC sites in December 2024 and piloted throughout 2025 and into 2026.
An evaluation of the deployment of the B2C app is planned to commence in December 2025. This evaluation will be conducted using an implementation science approach and it is aimed at understanding the acceptability of the B2C app among TLC partners (clients, Peer Health Mentors, and health care providers), and the effectiveness of the app. It will seek to identify opportunities to improve the app or aspects of how it has been implemented, as well as to determine what factors are important to the long-term success and sustainability of TLC and the B2C app.
To obtain quantitative data on health outcomes among TLC clients, we plan to obtain informed consent from TLC clients to extract their data from the B2C app and link this in an administrative data platform run by the BC Provincial Health Services Authority (PHSA) called PANDA. In PANDA, the TLC client data will be linked with public health and administrative datasets including hospital admissions, medication dispensation records, laboratory test results and death records. A de-identified linked dataset will then be used to track how TLC clients progress through care and to examine their health outcomes before and after enrolment in the program. We will also seek to use implementation science methods to evaluate the barriers and enablers encountered in linking the TLC client data with administrative and health data, and what the impacts of this linkage are for population health monitoring and public health surveillance.
A co-design process will be undertaken with clients, peers and health care staff in 2026 to incorporate their feedback and learnings from the third evaluation into the B2C app.
